FINANCIAL ARRANGEMENTS

In order to provide high quality leading edge care, we must also maintain a stable business structure. Even though, with changes in insurance coverage and economic fluctuations, we must adapt to our patient’s financial needs and their continued need for health care. In order to provide a convenient method of payment for patients we offer 6 methods of payment. The following credit cards are accepted:

VISA
MasterCard
Discover

We do accept all conventional insurance but do ask for payments of at least that equal to the co-payments, deductibles, or amounts over annual limits. These fees are normally requested at the time services are provided. Please come prepared to meet this request.

We will send you a monthly statement when a balance is due. A nominal interest rate may be charged. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Please remember you are fully responsible for all fees charged by this office irregardless of your insurance coverage. Your prompt remittance is appreciated. On occasion, we can make arrangements for a monthly payment plan but that must be done prior to the actual procedure. If you have questions regarding your account, please contact us at (626-440-0099). Many times, a simple telephone call will clear any misunderstandings.

We also are providers for a limited number of preferred provider plans (PPO) and health maintence plans (HMO). Currently we subscribe to the following care plans, but understand, these plans can change frequently:

Delta Dental
Facey Medical / PPO
Medicare Provider (medical)

Special financing is also available through a national healthcare financing company; Care Credit. Please speak with any of our scheduling coordinators for more information.

We of course are happy to provide care for cash for non-insured patients. Routinely, we ask for payment at the time services are provided for services up to $ 450.00, or 50% of the surgical fees if more than $ 450.00. Short term financial arrangements can be made in special situations on an individual basis. Please speak to our financial staff if you have special circumstances.
