PATIENT INFORMATION

WHAT TO EXPECT AT YOUR CONSULTATION

Initial Consultation

Your initial appointment will typically consist of an in depth consultation explaining your diagnosis and treatment options. This may be performed by a combination of our highly qualified nursing staff and our doctors.   Occasionally, surgery can be done the same day as the consultation. However, a complex medical history or treatment plan is best served with a separate evaluation and consultation appointment followed with a second appointment, to provide treatment on another day. We generally prefer to have preoperative consultations at a prior separate visit.  This format better informs and prepares you for your procedures.

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral request (if any) and any X-rays if applicable
  • A list of medications you are presently taking
  • Knowledge of your health, pertinent medical problems, and your treating doctors
  • If you have medical or dental insurance, please bring the necessary completed forms. This will save time and allow us to help you process any claims properly.

PLEASE NOTE: For your convenience, you may register prior to your appointment On-Line. To do so, please click on "Patient Registration".

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

A pre-operative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. For adults, please have nothing to eat or drink 8 hours prior to your surgery. For children (under age 10) this can be reduced to 6 hours prior to surgery. You will also need a responsible adult to drive you home.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, heart murmurs, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant (blood thinner) therapy, holistic supplements, etc.)

Radiographs (x-rays)

If your dentist or physician has taken x-rays, you may request that they forward them to our office to avoid the need for further films. If there is not enough time, please pick them up and bring them to our office. Please understand, that occasionally films from your general dentist are not adequate. If additional films are necessary, they can be taken at our facility.
